New Patients

 

WELCOME TO OUR PRACTICE

Our office welcomes new patients.  Please register through the Practice Portal to access new patient forms, access appointment requests, patient messaging with the office staff, and ability to pay your bill online.  Please do not hesitate to contact our office at (316) 685-0559 should you have questions or need additional information.

On the day of your first appointment, we ask that you arrive to the office 30 minutes prior to your appointment time so that we may complete the registration process and the nurse may review your health history.  Please complete and bring the forms that are available here on our website, or the forms that were mailed to you.  Please bring your current insurance card as well as a government issued photo identification card.  Please bring a complete list of medications and vitamins/supplements that you are taking to include medication name, dosage, and directions.  If your insurance requires a referral or copayment, it will be necessary for you to have these during check in as well.
